1) Star Kids Commitment to the Student and Parent and the
Student's and Parent's Responsibility to Star Kids
Star Kids Commitment to the Student and Parent
If all the terms of the Parent Agreement and Junior High and
High School Student Agreement (for students Grade 6-12) are
met each year, Star Kids plans to provide the following assistance
until the student graduates from high school. Once enrolled
in the program and if the parent agreement guidelines are followed,
Star Kids plans to follow each student with tuition, uniforms,
tutoring, after school programs and summer programs until the
student graduates from high school.
- Up to $3,500 per year of tuition assistance;
- $150 gift certificate per year for uniforms;
- Up to $300 per year of after school program assistance;
- Up to $300 per year of summer program assistance;
- Up to $150 per year for high school books.
- Field trips;
- Tutoring.
Star Kids support ends at high school graduation. Star Kids
does not provide assistance for college tuition and expenses.

- I will make sure I know when my child's school day starts
and ends and will make sure my child arrives at school every
day and on time. (Excessive tardiness or absence from school
(10 days or more) may be cause for dismissal from the Star
Kids Program.) I understand that my child must follow school
rules and treat the faculty and other children with respect.
- I will call the school as early as possible if my child
is absent from school and write a letter of explanation
about the nature of their absence when they return to school.
- I understand that Star Kids does not provide lunch and
I must provide a lunch for my child each day.
- I will make sure I understand when the first day of school
is and at what time and where the bus will pick up my child.
If I don't know, I will call the school directly.
- I understand that my child must maintain a C+ or better
average in order to remain in the program.
- I will check my child's backpack each day for notices
from the school and make sure I allow time for my child
to do their homework and will help them if necessary. I
will allow ample time to help my child prepare for long
term projects such as book reports and science fair projects
and if I can't do it I will ask someone at school or my
child's mentor for help.
- My child and I will write a thank you note to their sponsor
and mentor.
- I will follow school rules, cooperate with the school
authorities and will attend Parent-Teacher conferences.
I will take advantage of services recommended by the school.
- I will fill out financial aid forms from the school or
from the Diocese for the St. Mary's Fund and Frances Warde
Fund. I will be responsible for any fundraising the school
requires.
- If the school has religious requirements, I understand
that my child must follow the requirements.
- I will make sure I have a school calendar so I understand
when all the events are. I will volunteer at school and
plan to attend as many the school functions as I can.
- I will contact Star Kids if I need financial help with
tutoring or field trips.
- I will try to attend the two Star Kids parties- the Skating
With Santa and End of the Year Party.
- I will not ask my child's mentor or sponsor for money,
food or services (such as transportation).
- I will let Star Kids know of any change of address, phone
number or family circumstances and will return calls promptly.
I will cooperate fully with the Star Kids staff.
I agree to comply with the above Star Kids Program Parent's
requirements.

The goal of Star Kids is to help you graduate from high school.
Our expectations are high because we want to make sure that
you are well prepared for higher education in order to get and
keep a good job and to be successful in the real world. A Star
Kids Scholarship is a wonderful opportunity, but it is also
a privilege. It is a special program and therefore has special
rules. In order to keep this scholarship you must agree to the
terms of the following Star Kids Junior High and High School
Student Agreement. If the terms of the agreement are not met,
you will be required to attend a meeting and you may be placed
on probation and/or may be dismissed from the program at anytime
during the school year.
1. I will make sure I attend the first day of school is and
if I do not know the date, I will call the school directly to
find out. I will be on time for my ride to and from school each
day.
2. I will make sure I know when my school day starts and ends
and will make sure I arrive at school every day and on time.
I will not be tardy more than 10 times per year. I will not
be absent more than 10 days per year. I understand that excessive
tardiness and/ or absence from school may be cause for my dismissal
from the Star Kids Program.
3. I will remind my parent to call the school as early as possible
if I am absent from school and to write a letter of explanation
to the school about the nature of my absence when I return to
school.
4. I understand that it is my responsibility to make up any
schoolwork if I am absent from school for any reason. I will
remind my parent to contact my school and pick up schoolwork
if I am absent for more than two days in a row. I will notify
Star Kids if I am absent for more than two days in a row and
will send Star Kids the reason for my absence and a copy of
the doctor's note if I am absent for two or more days.
5. I understand that I must follow school rules and treat the
faculty and other students with respect. I will take advantage
of services recommended by the school.
6. I understand that Star Kids does not provide lunch and I
must provide for my own lunch each day.
7. I understand that I must maintain a C+ or better average
in order to remain in the program. I understand that Star Kids
does not pay for summer school.
8. I will bring home all school communications to my parents.
9. I will complete and hand in all my homework and schoolwork
on time. I will ask for and attend extra help sessions if necessary.
I will allow ample time to prepare for long term projects such
as book reports and science fair projects and if I can't do
it I will ask someone at school or my mentor for help.
10. I will be honest with my schoolwork and homework. I will
not copy anyone's work, plagiarize or cheat.
11. I will not use illegal drugs or alcohol.
12. I will write a thank you note to my sponsor and mentor.
13. I will be responsible for any fundraising the school requires.
14. If the school has religious requirements, I understand that
I must follow the requirements.
15. I will make sure I have a school calendar so I can understand
when all the events are. I will plan to attend as many the school
functions as I can.
16. I will contact Star Kids if I need help to find a tutor.
I will be on time for tutoring sessions and I will work cooperatively
and respectfully with the tutor.
17. I will contact Star Kids if I need financial help in order
to attend school field trips. I will attend all school sanctioned
field trips and wear appropriate clothing on these trips.
18. I will volunteer to help at the two Star Kids parties- the
Skating With Santa and End of the Year Party.
19. I will not ask my mentor or sponsor for money, food or services
(such as transportation).
20. I will let Star Kids know of any change of address, phone
number or family circumstances and will return calls promptly.
I will cooperate fully with the Star Kids staff.
I agree to comply with the above Star Kids Program High School
Students Agreement requirements. I understand that if the terms
of the agreement are not met, I will be required to attend a
meeting and I may be placed on probation and/or may be dismissed
from the program at anytime during the school year.
